Working with families: We understand that transitioning to a new home is a significant step for disabled adults and their families. We are committed to ensuring that this process is not only smooth but also tailored to meet the unique needs and aspirations of each individual via:

• Personalized Consultations with families: The journey by scheduling personalised consultations with families. During these sessions, our experts engage in open and honest conversations to understand the specific needs, preferences, and concerns of both the disabled adult and their family members
• Comprehensive Needs Assessment: A thorough needs assessment is conducted to identify the level of support required, medical considerations, and any specialized accommodations that may be necessary. This process ensures that the new home is equipped to meet the unique requirements of the individual
• Collaborative Care Planning: We work closely with families to develop a comprehensive care plan that addresses both the medical and emotional well-being of the disabled adult. This plan serves as a roadmap for the transition process
• Transparent Communication: Open and transparent communication is a cornerstone of our approach
• Facility Orientation and Tours: We arranges facility orientations and tours for families to acquaint them with the new home. This includes introducing them to the staff, showcasing the living spaces, and explaining the various amenities available to residents.
• Emotional Support: Recognising that transitions can be emotionally challenging, we offer emotional support to both the disabled adult and their families. Our team provides resources and a compassionate ear to navigate the emotional aspects of this significant life change
• Community Integration: We go beyond just facilitating a physical move; we emphasize community integration. We work to connect the individual with social activities, support groups, and other residents, fostering a sense of belonging in their new environment
• Ongoing Support and Monitoring: The support doesn’t end with the move. Ebony Consult remains actively involved, providing ongoing support and monitoring the well-being of the disabled adult. Regular check-ins and assessments ensure that any adjustments needed are promptly addressed.

Our service is registered with and regulated by the Health Information and Quality Authority HIQA. The authority (HIQA) conducts regular inspections to ensure the service is keeping its key performance indicators.
